JJ Mobile Physiotherapy – Terms and Conditions
By attending an appointment with JJ Mobile Physiotherapy, you are acknowledging and giving voluntary consent for our physiotherapist to perform any necessary assessment and treatment procedures. You can withdraw it at any time through communication with our team.
These terms and conditions ("Terms") govern the provision of physiotherapy services by JJ Mobile Physiotherapy ("we," "us," "our") to patients ("you," "your") and apply to all appointments, treatments, and related services.
Treatment
- As part of your treatment plan, our physiotherapists may employ various techniques such as manual therapy, exercise, and education. These techniques are designed to help improve your physical function and alleviate pain.
- Before proceeding with any treatment, your physiotherapist will explain to you the potential benefits as well as the risks involved. You will have the opportunity to discuss any questions or concerns with your physiotherapist. It is important to note that you have the right to decline any treatment or technique that you may not be comfortable with.
Cancellation Policy
- We understand that circumstances can change, and you may need to reschedule/cancel your appointment. However, kindly note that due to our limited availability and the challenges of accommodating other patients on short notice,
- A 50% fee of the scheduled session will be charged if notice is given less than 24 hours.
- If we receive less than 12 hours of notice before the appointment or if there is a no-show, 100% of the scheduled fee for that session, including travel, will be charged.
Payments
- Payment is due at the time of the appointment, unless we have agreed to invoice your representative, NDIS plan manager or Home Care Package coordinator.
- We accept cash, debit/credit card as payment.
- Private insurance may be able to claim on the spot using Medipass. Please visit ‘Claims and Payments’ on our HOME page.
Privacy and Confidentiality
- We are committed to protecting your privacy and confidentiality. We will collect and use your personal information for the purposes of providing physiotherapy services and related purposes only. Please visit our Privacy Policy for more information.
Liability and Indemnity
- We will not be liable for any loss, damage, or injury arising from the provision of our services, except where such loss, damage, or injury is caused by our negligence.
100% Money Back Guarantee
- A refund request must be raised within seven days from the day of your appointment.
- The 100% Money Back Guarantee applies only to the session fee. Any additional costs, such as travel, parking, and toll fees, are non-refundable.
- Any refund will be made in the same manner which you paid at the time of the service.
General
- We may amend these Terms from time to time. Any changes will be effective immediately upon posting on our website.
- These Terms are governed by the laws of Victoria, Australia, and you agree to submit to the exclusive jurisdiction of the courts of Victoria, Australia.
